We have a new and exciting opportunity for a Project Worker within our recently commissioned Hospital Discharge service which provides emergency bed provision for individuals with complex needs who are being discharged from a hospital or specialist setting.
This is a new initiative, set up in conjunction with Cheshire East’s Housing Options Team and local mental health hospital units and services who have experienced problems in placing homeless persons with mental health problems when leaving mental health hospitals or units.

To be successful in this role as Project Worker you will have knowledge and understanding of the needs of vulnerable and socially excluded people and have specialist knowledge of the specified client group, treatment pathways, and advanced care planning, together with specialist knowledge of the mental health inpatient and community services within the area and their roles in treatment and support of individuals.  You will also need to possess good written and verbal communication and IT skills; have the ability to maintain accurate records, together with great organisational and time management abilities.
